Deaths

Australian Death Registration Forms

The Registry uses the details from the death registration record to produce a NSW Death Certificate. Funeral directors are usually responsible for registering the death within seven days of the burial or cremation. The Notice should serve as a prompt first advice of death, which is later matched with the Death Registration Statement (from the funeral director) and the Doctor's Certificate of Cause of Death in a verification process. If the matching paperwork is not received, the Notice also serves to alert BDM that a death has occurred but the body has not been properly disposed of, or the funeral director has not registered the death.

Births, Deaths and Marriages Registration Act 1995 Registration forms of births, deaths and marriages

The Registry can only produce a Death Certificate if the death has been registered. Details of the death must have been lodged at the Registry for them to be recorded. Funeral directors are usually responsible for registering the death within 7 days of the burial or cremation.
